Once we have verified what you are looking for, as well as a method of payment (i.e. direct pay or a participating insurance plan), we will email you a link to our online portal, where you can complete new patient paperwork. 

Once you have received the email concerning this, you will be able to set up an account and password, as well as complete all pertinent documents online located at the website address listed below. Once this is complete, if using insurance, we will verify your benefits, deductible, and determine whether your plan is in or out of network with us. Once this information has been determined and communicated to you, we will be able to make you an initial appointment with one of our providers.
